Transcript requests
can be made via email, fax or mail along with the following information:
- Name
while in attendance at the HS
- Date
of birth
- Year
of graduation or year of withdrawal
- What
your request is: how many official or unofficial transcripts are needed
- Whether
they are to be picked up or mailed
- If
mailed, include address or addresses
- Sign
and date your request
There is a $5 per
transcript fee. If transcripts are being picked up at the HS, fee can be
paid at that time. If anyone other than the student is picking up the
transcript, please indicate their legal name. If transcripts are to be
mailed to you or to schools, please send a check payable to SMHS to the HS
address listed below
